Let's say for arguments sake that the insurance company PRE-APPROVES you for care after a medical procedure, and then says that you where really not covered for it, and you owe them $50,000? Since they pre-approved it they "insurance company A" HAVE to pay for the services and you DON'T have to, BUT if you pay EVEN A PENNY to them OF ANYTHING OF THAT 50k? Then you have assumed the responcesibility for that debt, and they are off the hook.
